I found that Google cached my old github account name, with full name and it's displayed o your site here: http://issues.joomla.org/user/645#
Who I should I contact and ask for removing it? All user data and (especially) photo?

Maybe we should handle this in a way which involves less human interference
Do you have any links to issues where the user was involved on GitHub to see how they handle this?
I think we could set up a piece of syncing code that might run on demand or periodically.
Their one action on the tracker was a vote on an issue. There was no activity that involved a sync to GitHub other than to log in and retrieve the user data.
