Summary of Changes
Missed it when it went to 3.9. This moves some classes and router calls from plugin class to layout.
Testing Instructions
Enable Content - Page Break plugin. Set Presentation Style to Pages. View a multi-page article. Inspect table of contents.
Expected result
Table of contents works OK.
