Steps to reproduce the issue

Asked for by @richard67 here #33631 (comment) - we already did this once recently (the checking of buttons order) but it seems there are several inconsistencies again

In documenting everywhere we use a modal will make this kind of thing easier in future when we want to test modal based views.

1. Document the places we use modals

2. Check and change to be consistent, the layout of buttons in the following order (note not all buttons will be on all places!)

[Anything Else] [Insert] [Process] [Select] [Save] [Save and Close] [Cancel] [Close]
