Missing 'administrator' part in the form's action of the admin login page. The form's action has value '/index.php' instead of 'administrator/index.php'.
Fix file
defines.php --> Joomla\includes\defines.php
uri.php --> Joomla\libraries\joomla\environment\uri.php
default.php --> Joomla\modules\mod_login\tmpl\default.php
