@Joomv Yes! I had ExpiresDefault enabled because it is better for performance in combination with caching. I removed these headers and the problem is solved now. Thanks for that!
But... It still is an serious problem. On a Dutch Joomla! Support Form (joomlacommunity.eu) are many people with the same issue.
So is this a problem caused by that server settings or is there anything we can do to prevent it?

@PhilETaylor at least when you have this directive enabled it can be confirmed 100% of the time (or at least I could today)
The bit I dont understand is that I can confirm it 100% of the time if I click on the article title in the manager list and I can confirm it 0% of the time when I select the article from the toolbar

Have you got ExpiresDefault enabled in your htaccess? This will cause this. It stopped doing it for a while in 3.0 but started again in 3.5. All the other Expires settings are fine, just the default causes the issue.
